FEES

You may be responsible for all or part of your session payment, even if you have insurance. Please be prepared to make your co-pay or your portion of the fees at the time of your appointment. If you are not sure how much you will owe, please contact your insurance provider directly. Usually you will find a customer service number on the back of your insurance card.

We accept cash, check and debit/credit payments (including HSA and flex benefits). To make things easy, you can sign up for autopay and have your session co-pays/fees automatically processed in our secure portal. Saves time and a stamp.

Private Pay

 

You can pay for therapy sessions directly without using insurance.  If you would like more information about private pay (self pay), contact our office at 319-239-3533.

Insurance

Our providers partner with some major insurance carriers including:

  • Wellmark Blue Cross/Blue Shield

  • Midland's Choice 

  • Iowa Medicaid (Iowa Health link & hawk-i) including

  • Cigna - Midland's Choice

Insurance coverage varies by individual plan. We suggest calling the member services number on the back of your insurance card to learn about your specific mental health benefits through your health insurance plan prior to your first visit.

 

We are unable to tell you your specific coverage, co-pay or deductible until it is already processed through insurance at which time you will be responsible for the full fee.

 

Some plans require an authorization for specialty services prior to your first appointment, please call your insurance company prior to making your first appointment to obtain this authorization, especially if your insurance provider is not listed above.

Our providers and services are not covered by Medicare.
