FEES
You may be responsible for all or part of your session payment, even if you have insurance.
Please be prepared to make your co-pay or know your deductible
payment at time of service. If you have concerns covering the cost of co-payment or
full session, please call our office ahead of time or speak to your provider.
We accept cash, check and debit/credit payments (including HSA and flex benefits).
Private Pay
We offer the option of private pay for services for clients who wish pay with
cash, credit/debit card, or health savings account cards. If you would like more information about
private pay, contact our office at 319-239-3533

Insurance coverage varies by individual plan.
We suggest calling the member services number on the back of your insurance
card to learn about your specific mental health benefits through your health insurance plan prior to your first visit. We are unable to tell you your specific coverage, co-pay or deductible until it is already processed through insurance at which time you will be responsible for the full fee.
Some plans require an authorization for specialty services prior to your first appointment, please call your insurance company prior to making your first appointment to obtain this authorization, especially if your insurance provider is not listed above.
Our providers and services are not covered by Medicare but will utilize a reduced sliding scale fee for individuals on Medicare seeking services.
Employee Assistance Program (EAP)
Some businesses may utilize Employee Assistance Programs (EAP) for their employees.
EAP’s may offer 1-6 counseling sessions at no cost to the employee. Be sure to check with your
human resources representative to see if you qualify for EAP sessions through your employer & to see if they partner with our providers. We currently partner with several EAP providers but if your place of employment uses an EAP provider that we are not contracted with, you may be able to request approval of our providers.
Please have all authorizations approved for your scheduled provider prior to your first appointment and be prepared to give that authorization number when you fill out new client paperwork.
